Physician Select Vitamins, LLC DBA Uscriptives (“us,” “we,” or “Company”) are committed to providing satisfactory products to our customers. If you are not satisfied with your product, we are here to help.

RETURNING AN ITEM BY MAIL

If you purchased a product from www.uscriptives.com and need to return it, here are the basic steps:

  • Pack the item securely in the original package, if possible.
  • Find and print your digital packing list, which you can find in your email, and enclose with your item. Be sure to keep this email for your records. If you cannot print out your digital packing list, please include your order number, billing name, and address with the item on a separate piece of paper.
  • All products must be returned, unused or used, in good condition, in original boxes (whenever possible), and with all paperwork and accessories to ensure full credit.
  • All return shipping charges must be prepaid. We cannot accept Cash on Delivery (C.O.D.) deliveries.
  • For your protection, we recommend that you use UPS or Insured Parcel Post for your return. Unless your digital packing slip directs you to ship an item to a different address (and if so, follow such directions), ship the return package to:

  • Keep the Return Tracking Number from the package you are returning to ensure that the package is returned to the warehouse.
  • You can expect a refund in the same form of payment originally used for purchase within 30 days of our receiving your returned product. You will be refunded the shipping cost if the return is a result of a manufacturer error or a shipping error on our part.

POLICY

In order to better serve our customers, a Refund and Exchange Refund policy has been established. Customers can return their purchase with a valid sales receipt, www.uscriptives.com digital packing slip, or verification of purchase date, tender and amount paid within 30 days from the date of purchase for a 30-day supply, 60 days from the date of purchase for a 60 day supply, and 90 days from the date of purchase for a 90-day supply. Please call 1-800-778-7191 between 8AM and 5PM CST Monday through Friday to receive a Return Authorization (RA) number. No returns will be accepted without a RA number.

Refunds: refunds are given on products purchased with debit or credit card, used or unused, within 30 days of the purchase date, if you purchased a 30-day supply, with a valid sales receipt, www.uscriptives.com digital packing slip, or verification of purchase date, tender and amount paid within 30 days from the date or purchase. If you purchased a 60-day supply, then you would have 60 days to return the product. If you purchased a 90-day supply, then you would have 90 days to return the product.

Exchanges: If the customer returns non-opened, used or opened product, an exchange will be offered based on the amount paid (as shown on the sales receipt, www.uscriptives.com digital packing slip, or verification of purchase date, tender and amount paid). Exchanges will be given for a similar or like-kind product within 30 days for a 30-day supply, within 60 days for a 60 day supply, and within 90 days for a 90-day supply.

Note: The exchanged product is to be the same retail price as that of the returned product. If the exchanged item is a higher price than the returned item, the customer will then be required to pay the balance due.

DAMAGED/DEFECTIVE ITEMS

If you receive a damaged or defective item or there is extensive damage to the packaging, contact a Customer Service Representative within 30 days of delivery at www.uscriptives.com. Please supply the Representative with your order number and item number from your original confirmation e-mail. The Representative will also need your e-mail address and phone number. The Company will make every reasonable effort to replace the item in a timely manner. We cover shipping charges if there was damage to the item due to shipping.
